The three-family residential building at 2136 Lacombe Avenue in Castle Hill, owned by Nirmala Santiago, has experienced several maintenance and safety issues since 2021. A significant concern arose in June 2021 when multiple complaints about the fire escape were filed with both 311 and the Housing Preservation and Development (HPD) department, though these complaints were ultimately closed due to lack of access for inspection.

In April 2021, the Department of Buildings inspection revealed multiple violations in Apartment 1 on the second story, including issues with a defective bathroom door, leaky bathroom faucet, and broken ceramic floor tiles in the kitchen. These violations were classified as Class A and B, indicating varying degrees of severity, but were certified as resolved by June 2022.

More recently, in July 2024, there was a complaint filed regarding an illegal conversion of the building, with a final inspection attempt occurring in March 2025. Unfortunately, this inspection could not be completed as the inspector was unable to gain access, marking this as the second final attempt to verify the complaint. While the building has seen improvements in resolving its earlier maintenance violations, the unresolved fire escape inspection and the more recent illegal conversion complaint suggest ongoing regulatory compliance challenges, particularly regarding access for inspections and potential unauthorized modifications to the building's configuration.
