The three-story, four-unit residential building at 628 East 16th Street in Flatbush, owned by Teddy Mann, has experienced several significant incidents over the past few years that merit attention. Most notably, in early 2021, the building faced multiple heat and hot water emergencies, with complaints filed on February 6 and February 15 about the entire building experiencing these issues. Unfortunately, Housing Preservation and Development (HPD) was unable to gain access to conduct inspections in response to these complaints.

The property also underwent construction work in 2020 that resulted in two Oath Violations on April 2 and May 14, showing potential unauthorized alterations. Further complicating matters, there are active Environmental Control Board (ECB) violations from 2020 concerning the building's front facade. One violation, issued April 17, 2020, concerns unpermitted alterations to second and third-floor windows which were reportedly reduced in size by approximately 32" by 22" each through brick filling.

Another active violation, from May 14, 2020, indicates that a required certificate of correction, dating back to 2017, had not been submitted, resulting in a $6,250 penalty. The building's rodent inspection history shows an initial report of rat activity on June 25, 2021, followed by a compliance inspection on August 3, 2021, which passed, suggesting the vermin issue was eventually addressed. While some issues appear to have been resolved, the presence of active ECB violations from 2020 and the history of heat and water problems in early 2021 are noteworthy concerns for prospective tenants.
