The three-story, four-unit building at 101-12 67th Drive in Forest Hills, owned by Devanie Gajadar, has experienced several maintenance and safety issues since its construction in 1960. Most recently, in late 2024, multiple complaints were filed regarding window frame conditions and wall maintenance, including reports of mold that were categorized as emergency situations.

An earlier heat/hot water complaint from January 2024 was resolved promptly within two days. The building underwent an inspection in November 2024 that resulted in several violations requiring attention, including the need to post proper smoke detector and carbon monoxide detector requirements, maintain gas leak procedures notices, and address specific painting and plaster issues in Apartment 2's bathroom and other areas. A significant safety concern was also noted regarding a defective self-closing building entrance door, classified as a Class C violation.
