The two-family house at 115-22 126th Street in South Ozone Park, owned by Kairool Nagapen, was constructed in 1930 and has experienced several notable issues over recent years. Most recently, in the first half of 2025, there have been multiple complaints regarding smoking or vaping in prohibited areas, with three separate complaints in May and June 2025 that have been referred to the NYC Health Department for inspection.

The building had significant maintenance and safety issues in 2020, including multiple violations and complaints during March that year. These included problems with heat and hot water systems, plumbing issues (bathtub/shower problems), and pest infestations.

The Housing Maintenance Code Violations from March 2020 revealed several issues in the second-floor apartment, including a roof leak affecting the public stair area, pest infestation, defective faucets, and multiple maintenance concerns in the bathroom and kitchen.
