The two-family house at 2956 Rockaway Freeway in Bayswater, built in 1999 and owned by David Parker, has experienced several concerning maintenance issues over recent years, particularly in 2021/2024. Most recently, in December 2024, there was a heat/hot water complaint affecting the entire building, though inspectors were unable to gain access to verify the conditions. In 2021, the building faced multiple simultaneous emergencies requiring HPD intervention, including mold issues, problems with plumbing (radiators and toilets), and appliance failures (refrigerator and electric/gas range).

Notably, housing maintenance code violations were issued for apartment 1, including issues with mold in the bathroom ceiling, broken window panes, and problems with kitchen appliances and bathroom fixtures, though some of these violations have since been marked as closed. The building has a documented history of heat and hot water issues, with complaints in 2019 and most recently in 2024, as well as other infrastructure challenges such as a power outage in July 2021, and various appliance and plumbing problems throughout the years. While some violations have been resolved and closed, as of now there remain several open violations from 2021 related to bathroom, kitchen, and window issues in apartment 1 that have not been officially certified as resolved. Multiple instances of inspectors being unable to gain access to inspect reported issues suggest potential communication challenges between tenants and maintenance personnel regarding property conditions.
