The two-family house at 124A Hendricks Avenue in St. George, owned by Liliana Torija, was built in 2003 and consists of two stories. The most significant cluster of issues occurred in 2019, particularly in the basement apartment, which experienced multiple problems including pest infestations (mice), electrical wiring concerns, and various maintenance issues.

Four separate complaints were filed and investigated in March 2019, though no violations were issued in any of these cases. During the same period, a violation was documented regarding a broken glass pane in a kitchen window. An appliance-related incident involving an electric/gas range was reported in August 2019, but HPD was unable to gain access for inspection, leaving this matter unresolved.

Previous documents show a pattern of maintenance challenges in the basement apartment, including multiple rodent inspection failures in 2010 (six consecutive failures) before passing inspections in 2014, 2016, and 2017. The building has also had various repair needs documented, such as problems with plaster surfaces, walls, and water leaks in the third room of basement apartment 1.
