The two-family residence at 680 Leverett Avenue in Arden Heights, owned by Jiarong Chen, is a two-story building constructed in 1965. The property has a documented history of three notable incidents between May 2019 and September 2020, all of which were officially closed with varying outcomes.

In May 2019, there were two separate complaints - one regarding illegal plumbing work (including concerns about sprinkler/standpipe systems) and another concerning an alleged illegal conversion of the property. Both of these complaints were investigated on May 16, 2019, with the plumbing-related complaint resulting in no violations being found at the time of inspection, while the conversion complaint was marked as "Inspector Unable To Gain Access – 2nd Final Attempt."
