The 2-story, 4-unit multi-family residential building at 574 28th Avenue in San Francisco's Central Richmond neighborhood, constructed in 1924, has a documented history of fire safety and building maintenance issues primarily in the early 2000s and early 2010s. A significant set of fire safety violations was recorded in June 2002, including requirements to remove combustible storage or install automatic fire sprinklers, recharge fire extinguishers, and install a smoke detector in the lobby; these violations were ultimately abated by September 2002.

An additional set of violations in April 2011 called for the installation of properly maintained fire extinguishers and repair of deteriorated floor boards in the second-floor rear exit stairs, which were also resolved within approximately one month. The building underwent dryrot repairs in February 1985, as documented in building permits.
