The two-story, four-unit multi-family residential building at 570 26th Avenue in the Central Richmond neighborhood, owned by John T Nakabayashi Revoc Lv, was constructed in 1981 and has experienced several significant maintenance and safety events over its history. Most notably, in April 2000, the building underwent a comprehensive fire safety inspection that revealed multiple violations including egress obstructions, combustible storage issues, smoke detector deficiencies, and fire proofing concerns.

These violations were all successfully abated by June 19, 2000.
